KOHA

The Welfare Fund pays a koha of $4000 to the family on the death of a member; $2000 on the death of a member’s spouse/partner; and $2000 on the death of a members child under the age of sixteen years.

The member must be financial at the time of the death.

Applications for the koha must include

Members name (and membership number if known)

Employers Name

The relationship to the member of the person claiming the koha.

The Claimants address and phone number

A copy of the death certificate

Proof of age if the death is a child under 16
